Yippee kai yeah!

It had been a pretty depressing time lately.

Firstly, it was the death of Teoh Beng Hock.

Secondly, it was Yasmin Ahmad’s passing away. May she rests in peace, but most of all I wish someone can really continue her legacy of spreading the real message of 1Malaysia, unlike the one you see in NTV7, where a whole bunch of popular young entertainer and broadcaster going around quoting ‘1Malaysia’ as if you’re going to get much hippier or cooler, if you use the word 1Malaysia. Get real, man!!

In my mind, I was imaging a scenario of a boy going up to a girl.

Boy: Hi gal, 1Malaysia…

Girl: (Jumping up and down like nuts) WOW… That is so cool!!!

With the death of Sdr Teoh, thousand of questions were unanswered. Not like there wasn’t any mystery questions before ie. those of Unit Khas and Mongolian model.

Where lawless became a law, as one of my pal puts it
– it is a cowboy town.

From my own point of view, the whole state of Malaysia is slowly evolving (or already evolved) into a cowboy country.

I am sure the police or MACC had their own way of securing information. Just like the medical world, the police or army or any professional bodies, sure there’s this unwritten law, the common rules or the way of doing things.

“Please be reminded, in the army, there’s always 3 ways of doing thing. The right way. The wrong way. The army way.”

That was the quote from the movie ‘The General’s Daughter’. Sometimes fiction isn’t that far away from reality.

But whether it is the right way, the wrong way or the x way, there must be a limit, a self-regulation, a fail-safe key in which serve to prevent them from being compromised publicly and all.

From the obviously mysterious way of death creeping up to Sdr Teoh, undeniably, police or MACC had failed to do so. And the public had lost their confidence in the people who are there to serve and to protect. In medical term, ‘autoimmunity’ serves as the closest metaphor in this situation, and mind you, a disease of this nature can be pretty depressing, and most often control is the key of treatment, and rarely a cure may surfaced.

With the eroding trust and confidence of police or MACC, the public will have to face the growing crime rates, which is reasonable. When the cat is being punished, the mice will laugh their teeth off and help themselves to the big piece of cheese.

Conspiracy apart, the unanswered truth behind Teoh’s death will leave the current national leadership in shoddy shade, but then again, there wasn’t much that one can do given the fact, Teoh wasn’t the first mysterious death in Malaysia. I’m not saying all those mysterious deaths were linked but somehow it does sounds like a case of “you scratch my back, I scratch yours”

It is definitely a very difficult way of living, and you can never afford to be too careful. Carelessness comes with a big price, and at times, even your life may not even be enough to cover the cost.

At the darkest hour, probably the alarm bells are ringing loudly, so loud that it probably echoes through heaven and hell.

Hopefully, change will come to our salvation before everything is too late.

birthday reflection

Birthday. That was 28years ago. Spontaneous vaginal delivery with epidural anaesthesia.

Birthdays. Almost every year having it.
Somehow I got sick of this yearly celebration some few years back.

I know it isn't a norm for someone to hate his own birthday.
Well, I don't.
I just don't get too excited over it.

When I was small, my birthday was usually a big fanfare and all. My parents would organize birthday parties every year, which I had no memory of (possible sign of early brain atrophy). I am not excited so much being the star of the party but I hard know anyone back then, except somewhere at the back of my mind, they're all related to me. Somehow we all share the same blood lines, our antibodies doesn't vary much. Nonetheless, I am happy for those birthdays, it meant a lot to my parents. Those birthdays marked my process of growing up, and now I am all grown up.

Secondly, if you know me well enough, I am very bad with dates, especially birthdays. I mean remembering them. And when my friends remember my birthday, wishing me and celebrating with me, I sort of felt rather guilty that I can't remember theirs. In return, I don't feel quite right during my birthday. Yeah, I know. Things doesn't really work this way. For me, it does, sometimes.

Thirdly, I don't think I done too much on that day, 28 years back. Basically, my mom was the one risking her life, and used all her energy to bear down, and delivered me vaginally. Talking about the delivery, she made it sound effortless and painless as she was on epidural, but no matter what, she's the one who should be celebrated for today, not me. Nonetheless, my close friend back in Uni did pointed out to me that it is my presence that worth the celebration on that day. Maybe she's right.

Anyway, contrary to going out for recreational drug, booze, party and wild sex (which is prohibited given my current family and work condition), from this year onwards, perhaps, I will religiously reflect on this day, how had I lived my life in that 1year back.

A life not examined is not worth living.

Someone said that before. I sort of forgotten who said it. Maybe it is from the movie. Just, maybe. Or could it be from Socrates.

I guess with this reflection, I may just as well begin to love my birthday.
A day, I count my achievement, my joy and my blessings. Nothing beats blessings.

Personal

Last year, end of july, I was involved in a traffic accident. It was a car vs motorbike accident, with the post-call me behind the steering wheel. Luckily, the motorcyclist wasn't hurt badly, or else my guilt would have kill me. Sort of reminds me of my junior who passed away due to highway accident. Note to self - never ever drive post call without influence of caffeine.

During this year's Chinese New Year, I learnt how to play Cheh Ling and involved in the pork-poking session.

And lastly, today, I got to see the 2nd partial eclipse of my life, and it is on my birthday. Now, who is going to top that. Hah. It was pretty hazy initially, but the haze sort of thinned out, and got to see the eclipse, together with my dearest.



I swear, in this lifetime, if I had enough bucks and enough annual leave, I am definitely going to the place with full eclipse. At least once in my lifetime.

As with my dearest, just early last month, this is the first time, I brought her along, to my conference trip. Not that I had a lot of those trips, anyway. But true enough, Sabah is a place worth visiting, re-visiting and re-visiting. White water rafting and SCUBA diving there was a truly worthwhile experience.

Besides that, we got over 2 major obstacles, and the details of those obstacles would be P&C. Private and confidential.

Now that my department got back a bit of strength, I was able to get back Penang more often and joined more activities by Wei Pin and the gang - the getting fit part (hiking and all), and the getting fat part (eating and all).

It is just so nice to get back in touch with them.

Month of May, everything was falling apart. My dearest's beloved gma had urinary tract infection with severe electrolyte imbalance and my dearest down with Chikungunya - at the same time. It was my first time sleeping on the hospital floor (with carpet), as I found the chair rather ergonomically-challenged. Luckily, everyone is well and all healthy now. For once, I'd really help if everyone put away the wrongful thoughts of 'people usually go into hospital because their severely sick and due to severity of the illness, they won't make it'. Get real, man.

Passion

With time, experience and knowledge grew. That is expected of myself (in that matter, anyone), and I swear my hospy is a bloody good learning ground full of excitement.

August 2008, I was there, my first case of cord prolapse, and she progressed to deliver vaginally without any maternal or neonatal morbidity.

And few months back, March 09, I C-sectioned a lady with foot prolapse. Scary experience.

My surgical skills, although still pretty lame (this is true, not a humble statement), progressed further under the tutelage of Dr. M and Dr. J. Got a touch of doing hysteroscopy, cerclage, hysterectomy and amateur laparoscopics. Beside that in the everything-gone-wrong month of May, I got myself into a surgical complication after ERPOC, which I believe I will remember for life. Of course not forgetting, my Anaest colleagues who supervised my spinals.

Dr H. provided guidance in completion of my first research study write up, which was published in MJOG, Malaysian Journal of O&G. Not something worth being so 'kembang' about as it was poster presentation in OGSM congress, therefore by default, it's in.

I was pretty thrill that I nailed the March paper.

And shortly after, I was blessed to have the approval from HOD to go for the timely long-awaited Paeds NICU 1-month attachment. And like I said, my hospy is a bloody good learning ground full of excitement, and I just couldn't thank Paed Specialists and colleague enough for their hospitality and teachings.

Along the year that was, my HOD got admitted into IJN, but now already recuperating well, and all ready to go and hunt a few tigers. And along with his absence in leading the team, 'falling apart' would probably best describe the departmental running during that season, from the bottom up. Frankly-speaking, I was emotionally pretty tired and fed up, probably silently holding up situational vulgarities.

Beginning of the year, I was put to take care of the kids in the house - the house officers. It was not an easy thing, not because the HOs were crappy, but simply because i create my own headache as I was aiming to be as fair as possible and there were too many intruding 'suggestions' to my management. But still, I survived and as for now, my kids were holding up the system pretty well.

All Hindu temples will be closed during the eclipse period. People are urged not view the eclipse directly, and it is better not to travel during the period and stay indoors.

Those who were born during an eclipse should take even more care than others and those born under the zodiacs following the moon sign of Aries (Mesha), Pisces (Meena), Libra (Thula), Sagittarius (Dhanus) and Gemini (Mithuna) should also exercise caution this month.

But I think the best is to take ample precaution not based upon superstitious ground.

There are few ways. You can project the image of the Sun onto a piece of paper or a wall either by using pinhole projector (long box or cardboard) or telescope, or use a solar filter either the glasses type or attached it in front of a telescope. Only then, you can observe the Sun safely. You don’t really need a telescope to enjoy this.

Caution: Sunglasses, exposed film, x-ray film, polaroid filters etc. are NOT suitable to be used for Sun observation. Use only filters or glasses that are specially made for Sun viewing.

Remember this: Safety precautions always have to be taken for any observations involving the Sun. NEVER EVER look at the Sun directly without any safety filters. The consequences will be a damaged eye or blindness.

Please bear in mind that although the Sun will be partially covered, the remaining of the crescent Sun will still be intense enough to damage your eyes. It is NOT safe at all to look at the partial Sun directly.

Toe thumbs - Megan Fox and me

Pilo has this toe-thumbs, and everyone around him is always fascinated with it. Ok, not everyone.
Majority would generously pass it off as cute, by cute, it is defined as 'adorable but ugly'.
Some would simply say 'oh that's so cool'. I don't think it is cool at all, but somehow it has always been that special part of me.

So, back to the original Q, why this nic of pilocarpine?
Basically, he took on the nic 'pilocarpine' back in 2001, when he first entered uni, fresh bugger, impressive singer with lousy voice during orientation, and to keep his name easily remembered, one of his super-duper senior, Dr. Lim KS tagged him as pi-locarpine, where -locarpine sounds exactly like his full name.

During orientation? Doesn't that made him hate the nic more?
Not really.
1, Dr. Lim is a nice fella.
2, coincidentally, that front 'Pi' or π is a mathematical constant whose value is the ratio of any circle's circumference to its diameter in Euclidean space; and same value as 22/7, which is exactly his birthdate 22nd July.

He doesn't hate that nic at all?
A bit, when people started to spell his name as Lo Kar Pin, (the right spelling, kindly refer to this blogsite name) and whenever the whole class in the lecture hall giggled whenever the lecturer mentioned the word 'pilocarpine' during pharmacology class.

What exactly is 'pilocarpine'?
Pilocarpine is a muscarinic alkaloid obtained from the leaves of tropical American shrubs from the genus Pilocarpus. Basically it is a medication which constricts your pupils to combat close angle glaucoma and juices your mouth if you're suffering from dry mouth.

What do pilocarpine work as?
He's actually a medical nut who is obsessed with Obstetric & Gynaecology. As opposed to what public perceived as the pervertic subset of medicine, he totally digs the adrenaline rush of being in the heat of duty. Being there for the wonderful miracles of childbirth and keeping the mothers alive, in his firm belief, is the best job in the world.

Why stalk? Why this blog?
Life is not a written book you can pick up and read. It is a journal waiting for you to fill it's pages. And this is pilocarpine's life. This is his journey, his thoughts and a close record of it. It will probably come in handy when he wants to take nostalgic trip back in time. Who knows, this may eventually serve him well when he's old, senile and probably demented.

That's probably for me to know, and for you to find out. I am a stalker anyway, what kind of life do you expect me to have. OGSM, not orgasm III: banquet. ning baizura

On the 2nd last day of the congress, I was trying my best to visit some of the booth, while my dearest met up with her long, long, long... long time netfriend Stevenie, meeting up her for the first time.

Everyone was given a complimentary bird whistle, of which was encouraged to blow, instead of clapping to show recognition. Sounds kind of mediocre to the people not attending this banquet, but personally, we're all definitely having a marvellous time, although the major whistling did not attracts any birds.

The birds wouldn't have go near the hall, as the ceiling lamps were purely spooky. It probably looked like the ceiling of the Spider Spirit's home of the Journey to the West series.

And then, there was great line up of traditional dances and ceremonial display, with all their awesome traditional attire.

Of course, being seated together with Ern Huei and Daphne made this banquet even more enjoyable.

Then in night time, grand dinner. the banquet. I swear, the food wasn't better than the opening ceremony. Probably my standards just went up. Probably.

OGSM, not orgasm II: talk. party. Jolin

Initially, I was thinking this conference would be a walk in the park. But I was so wrong. It is jam-packed with lots of seminars and interesting symposium, and I definitely wished I could have split into 3 to attend 3 symposium that were held all at the same time.

This time, management had some setback. Basically, the hotel over promised, and first few symposiums was held in a carpark, with aircond vent blowing a rather well-warmed air, and the noise from the exhibition booth and the neighboring makan place.

For a consultant, or at least a specialist to be presenting in this kind of condition probably requires more than courage. Perhaps, it is professionalism.

The show must go on.

And indeed, as unpleasant as the condition of the symposium may be for the learning audience or crowds. The speakers were not the least bothered and instead of browsing through the slides like a disgruntled lecturers, they shared their knowledge as enthusiastic as ever.

One of them, Prof Georgiou actually not only promoting the latest innovation in fighting PPH, the expensive Bakri balloon, Prof also gave practical lesson on how to make a condom catheter balloon which seems to be a much more practical and cost effective item for our hospy.

To be honest, initially I was shocked to be sitting among the crowds in such a hot place, but learning was fun with all the new knowledge.

I guessed the management team got into a 'heated' (forgive moi pun) with the hotel people and the following symposium was conducted in a much conducive areas of the hotel. So much for the hotel team to 'accomodate' the changes.

Night time, initially OGSM was promising a street party, but at the same time, there was the promotional tour of Jolin Tsai in 1borneo itself.

And due to the fact that the streets going to be jam packed with cars and tattoo-laden, well waxed body, multiple colour-dyed hair, heavy celtic make-up teenagers and fans of Jolin, street party was converted to a beach party.

My dearest and I wasn't so much of a fan of Jolin, and so we decided to for the beach party.

And shuttle bus brought us there.

The minute I got to the seaside. My jaw dropped.

OGSM, not orgasm I : 5stars experience

A break-away from the working routine and a little get-away together with my dearest to this conference.

OGSM basically stands for Obstetrical and Gynecological Society of Malaysia. But to be called as simply 'orgasm' may seems offending for some, but in my opinion, that just so cool.

Going to this congress wasn't a easy task to take up the hill.

If not for my research study to be presented in poster, i may need take a long queue before going to any course. The only course that I ever will get to do will probably be the Prevention of Fall in Patients workshop which was a basically more directed to the nursing side, and we go there just to make the workshop more credible. What I learn from the whole workshop can probably be summarized and fully digested by a paragraph beautifully captioned over a piece of A4 paper.

Going to this conference probably I got on somebody's nerve. But somehow, caring too much for that kind of somebody kind of make me bit pussy. pussy cat, ok. not the other pussy.

Anyway, big hugs to my mom for covering me and my dearest to take on family 'on calls' for that whole week.

Not to mention, Solvay for sponsoring my registration fee for the conference.

This would be my second visit to the land below the wind.

And OGSM conference is in 1borneo, and late booking left me with Tunehotel. I had always wanted to taste its '5stars' sleeping experience.

Due to lack of phone line from the room to the counter, I had to take a lift down to the lobby and asked them politely to change another room.
Receptionist: Wait a minute. I will ask my technician to go and see first.
Me: I think better change a room. Fixing it may take a very long while.
R: No. I will ask my technician to go and see first.
(waited for 10mins. called my dearest up there. technician not seen anywhere.)
Me: Excuse me, can we just change room. I just got into the room, didn't even move in yet.
R: No, wait first. my technician is on the way.
(waited another 20mins. technician finally there. walkie-talkied to the receptionist)
R: Berapo lamo?
Technician: agak lamo...
R: begini.... ah.
Me (interrupting): Kan dah bagitau?
R: Ok, we'll change another room.
M: Thank you. (in the most sarcastic tone ever. ever)

All freshen up, and ready to go for lunch after '5stars sleeping experience' delayed our lunch to tea time.

1borneo is a rather nice hyper mall. Perhaps a little bit too high end for Sabahans.

Ended up having dinner at the Wagamama Japanese Restaurant with 50% discount with my credit card. Typical sushi and sashimi food, but with a great bargain. After so many weeks, I guess that was my first step in damaging my personal weight loss program.

The next morning, I woke up early, purely because the sun was well up early in the morning. All bright and sunny 8am in the morning.

And I took a nice view of the landscape outside my room.

Then I went to the pre-conference workshop with Prof Kula. This workshop was well run. Prof Kula not only was dynamic, he was giving us a lot of practical points. I think after the workshop, my CSF probably went all dried up.

Then in the evening, the conference finally opened, with the standard itineraries and big shots giving speeches, plus special this time around, is the officiating of OGSM website.
